MyChart is a secure online portal that allows you to access your Sonoma Valley Hospital’s electronic medical record and send messages to your doctor directly without going to the hospital or picking up the phone.
Activation codes will be sent to SVH patients that have given consent to receive notifications by email or in a text.
Patients that already use MyChart will be able to link the SVH MyChart to their existing MyChart account. If you do not see the Sonoma Valley Hospital logo in the upper right-hand corner, go to your Learning Library to Link Accounts.
Patients with questions about MyChart can call the MyChart Patient Engagement Team at 1-833-395-2035 Monday – Friday: 7:00 am – 7:00 pm and Saturday – Sunday: 9:00 am – 5:00 pm. Interpretation services will be made available.
