Sonoma Valley Hospital Recognized For Quality Performance By Partnership HealthPlan of California

Sonoma Valley Hospital has been recognized by Partnership HealthPlan of California (PHC), a nonprofit private healthcare organization, for scoring at the top of their Hospital Quality Improvement Program. SVH was one of just three hospitals in Northern California to be recognized for achieving a perfect score of 100%.

In making the announcement in December, PHC Chief Medical Officer Dr. Robert Moore said, “PHC is pleased to recognize the hard work of Sonoma Valley Hospital to deliver high-quality care to each individual. The past 18 months have been extremely challenging and to push through to ensure the communities are as healthy as possible is nothing short of heroic.”

The PHC Hospital Quality Improvement Program, established in 2012, is a pay-for-performance program for hospitals serving the 600,000 Medi-Cal members in its service area which covers 14 Northern California counties.

“We are proud of this recognition because our entire team works mindfully every day to provide our patients with the care they would want for their own family and loved ones. This ‘perfect’ score acknowledges the team’s extraordinary efforts, especially during the COVID-19 pandemic,” said John Hennelly, Sonoma Valley Hospital CEO.

Partnership HealthPlan of California is a nonprofit community-based healthcare organization that contracts with the state to administer Medi-Cal benefits. PHC provides quality care to over 600,000 Medi-Cal members. Beginning in Solano County in 1994 PHC now provides services to 14 Northern California counties – Del Norte, Humboldt, Lake, Lassen, Marin, Mendocino, Modoc, Napa, Shasta, Siskiyou, Solano, Sonoma, Trinity, and Yolo. The goal of the Hospital Quality Improvement Program is to improve the quality of care provided to PHC’s Medi-Cal members by offering participating hospitals financial incentives in exchange for meeting selected performance targets. The measures hospitals report on are selected and designed in collaboration with PHC’s hospital partners and represent a full set of quality measures including readmissions, advance care planning, clinical quality, patient safety, operations and efficiency. The other two hospitals that scored 100% by PHC are Howard Memorial (Willits, CA) and Redwood Memorial (Fortuna, CA)).
